Creating contact form for your blog make your identity in the world of internet. It not used to know about the author, rather to have a two communication between the blog author and blog visitors. It would also create a trust on your blog. There are multiple ways to create form. I am going to teach you how to add form using Google docs. Google docs offers you to create your own form and embedded it on your blog for free.
Step 1
Go to www.docs.google.com. Sign in using your google account.
Step 2
Click create new button, then click Form and you would be directed to form edit page.
Before you could enter your details. Follow the instructions
Step 4
Click Add item button and select paragraph text, as we are going to create a contact form.
Enter Name in the sample question 1 text box and enter Email in sample question 2 text box, from edit option and select done. Do the same step for sample question 2 text box.

Type Contact Me in question title text box and help info in help text box. Check the required question check box to make the text value mandatory. Select paragraph text in question type drop down box, which is the default option.
Step 6
You are almost done. Click More action button and select Embed and copy the code.
